bb reports

Reproduction and root-cause reports for get-bb/bb issues. Each report has a minimal repro, screenshots where visual, root cause with permalinks, a first-principles fix proposal, and a hostile review of any linked PR. Every report was independently re-executed by a second agent (see the Verification section in each).
